Where applicable, 5Axis Health Inc acts as a Business Associate under HIPAA and processes
Protected Health Information solely in accordance with applicable Business Associate Agreements and
law.

Health Inc related to account activity, system alerts, security notifications, or support communications.
You may opt out at any time by replying STOP. Message and data rates may apply.
